Overview
Bile duct (biliary) injury is a serious complication that can occur during gallbladder surgery, when the bile duct is accidentally cut, clipped, or narrowed. It leads to bile leakage or blockage. Early recognition and expert repair at a hepatobiliary centre are critical for the best long-term outcome.
Causes
- Injury during laparoscopic cholecystectomy
- Difficult anatomy or severe inflammation
- Bleeding obscuring the surgical field
- Misidentification of the bile duct
- Thermal injury from cautery

- Acute or severe gallbladder inflammation

Treatment Options
Surgical Treatment
Management depends on the type and timing of injury. Minor leaks are often treated with ERCP and stenting. Major injuries require surgical reconstruction — most commonly a Roux-en-Y hepaticojejunostomy, where the bile duct is joined to a loop of intestine. Recovery
